Incident Reports
A fire or EMS incident report is generated by the company each time a piece of apparatus is dispatched to assist with a fire, medical emergency, hazardous materials incident, rescue or other dangerous situation. To request a copy of a fire or EMS incident report, complete the appropriate report request form below and carefully follow the printed instructions, including the instructions for returning it by mail/ email. There is a $25.00 fee when requesting fire reports and a $35.00 fee when requesting EMS reports. Please send all request to [email protected]
